Bursitis and Impingement
A bursa is a small fluid-filled sac that helps reduce friction. If the space under the top of the shoulder gets crowded, the bursa and nearby tendons can become irritated, especially with repeated reaching and overhead work.Rotator Cuff Tendon Irritation or Partial Tears
Tendons can flare after repetitive lifting, poor mechanics, or a sudden increase in training volume. Some people do have tears, but chronic irritation can be just as disruptive when the shoulder never fully regains strength and coordination.
Labral Stress and Instability
The labrum helps deepen the socket. When it is irritated or the shoulder is less stable than it should be, symptoms may include catching, clicking, or a shaky feeling during pressing, push-ups, or throwing.
AC Joint Overload
The joint at the top of the shoulder can become tender with bench presses, dips, heavy carries, or repetitive golf and racquet sports patterns.
Biceps Tendon Pain
Front-of-shoulder pain often comes from the long head of the biceps tendon. It can become irritated with pulling movements, curls, and overhead activity.
Frozen Shoulder
This is more than tightness. The capsule stiffens, and range of motion shrinks, making everyday movements like reaching overhead or behind your back feel stubborn and limiting.
Referred Pain From the Neck
In some cases, the shoulder is not the primary issue. Neck-related nerve irritation can create pain, tingling, or weakness down the arm that feels like a shoulder problem.
